Dermal filler 101: Everything you need to know before your first treatment
If you’re considering dermal filler for the first time, it’s normal to have questions. What exactly is filler? How does it differ from Botox? How long do results last? And perhaps most importantly, how do you avoid looking overdone?
This guide covers the basics of dermal filler treatment, including the different products available, the areas they can treat, what to expect at your appointment, and how long results typically last:

What is dermal filler?
Before-and-after photography is a legitimate tool. A well-documented set of imaDermal fillers are injectable gels placed beneath the skin to restore volume, smooth lines, or refine contours. Faces lose volume over time, and fillers are a way to put some of it back subtly, precisely, and in ways that are reversible if you change your mind.

HA fillers explained in plain language
The vast majority of dermal fillers used today – including every product at OrangeTwist – are made from hyaluronic acid, or HA. Hyaluronic acid is a substance your body produces naturally. It lives in your skin, joints, and connective tissue, and it is responsible for keeping skin hydrated and plump.
When HA is formulated into a filler gel and injected, it integrates with the surrounding tissue and acts like a cushion. It adds volume where volume has been lost, and it does so in a way the body recognises. Over time – typically six to eighteen months depending on the product and area – the body gradually breaks it down. At that point, a touch-up returns results to where you want them.

The Restylane Family
Restylane is another major HA filler family, manufactured by Galderma using a different cross-linking process called NASHA (Non-Animal Stabilized Hyaluronic Acid) and XpresHAn technology. Restylane products have a slightly firmer, more particulate texture compared to Juvéderm, which some providers prefer for certain applications, particularly areas that benefit from more lift.
- Restylane Lyft – Used for cheek augmentation and mid-face volume. A structural product, similar in application to Voluma.
- Restylane Contour – Designed specifically for cheek definition and mid-face contouring.
- Restylane Kysse – A lip filler with XpresHAn technology, designed to move naturally with the lips during expression.
- Restylane Defyne / Refyne – Softer products for moderate lines and folds. Defyne is denser; Refyne is softer.
- Restylane with Lido – A general-purpose HA filler with lidocaine for enhanced comfort.

The Revance RHA family
RHA stands for Resilient Hyaluronic Acid. The RHA Collection, made by Revance, uses a gentler manufacturing process than traditional HA fillers – one that preserves more of the natural HA chain structure. The result is a filler that is designed to flex and move with dynamic facial expressions rather than staying rigid between them.
This makes RHA particularly well-suited to high-movement areas: the lips, the lower face, and the nasolabial folds. Some clients notice RHA feels more natural during expression than traditional fillers in these areas – there is less of a sense of something sitting there.
- RHA 2 – The softest formulation. Used for fine, dynamic lines close to the surface.
- RHA 3 – A mid-weight product for moderate folds and lines.
- RHA 4 – Denser, for deeper folds and areas needing more structural support.
- RHA Redensity – Specifically formulated for the under-eye and tear trough area.
How your provider picks the right dermal filler product for you
Selecting the most appropriate filler is an important part of treatment planning. At your You Review, your provider will look at the areas you want to address, assess your facial anatomy, consider how much volume loss has occurred and where, and factor in your goals – whether that is very subtle, or something more defined.
Softer products go where softness is needed: close to the surface, in areas that move a lot, in fine lines. Denser, more structural products go deeper: into the cheeks, the jawline, the chin. No two faces are the same. Treatment recommendations are tailored to the individual rather than based on a standardised approach.
What issues does dermal filler treat?
Fillers are versatile, but they are not universal. Here is a plain-language overview of the most common treatment areas – what can be addressed, and what realistic results look like.
Cheeks and mid-face volume
Volume loss in the mid-face is one of the most common changes that comes with age, and one of the most impactful to address. When the cheeks deflate, everything below them tends to follow – leading to hollowness under the eyes, heavier nasolabial folds, and a face that reads as more tired than it used to.
Dense, structural fillers placed in the cheek and mid-face can restore that foundational volume. The effect is often a subtle lift across multiple areas, not just the cheeks themselves. Results from structural cheek filler tend to be among the longest-lasting – often 12 to 18 months or more.
Jawline and chin contour
Jawline definition is one of the fastest-growing filler applications. Dense fillers placed along the jaw create sharper definition; placed at the chin, they add projection and help balance facial proportions. This is a particularly effective area for clients who want a stronger profile or more symmetry without surgery.

Lips
Lip filler is probably the most visible and most discussed filler application – which makes it the one most people have opinions about. Both are the result of volume loss in the mid-face, combined with the effect of gravity and repeated expression over time.
Filler can soften these lines significantly. In many cases, addressing the mid-face volume loss that is causing them is more effective than treating the lines directly – which is why your provider may recommend both.
What to expect from your first dermal filler treatment at OrangeTwist

- The You Review

A few things worth mentioning at your You Review: any medications you are currently taking (especially blood thinners like aspirin or ibuprofen), any previous cosmetic injections, cold sore history if you are considering lip filler, and whether you are pregnant or breastfeeding.
- The dermal filler injection process
Before injections begin, your provider will apply a topical numbing cream to the treatment areas. This sits for a few minutes while the area becomes numb. Most filler products also contain lidocaine – an anaesthetic that further numbs the area as it is injected, so comfort improves as the treatment progresses.
Your provider will mark the injection points, then work through the treatment systematically. The process takes 30 to 60 minutes depending on how many areas are being treated. Some providers use a cannula (a blunt-tipped tube) for certain areas rather than a needle – this reduces bruising risk and is often more comfortable.
After injections, your provider will assess the result, mould the product as needed, and walk you through aftercare before you leave.
- Comfort and ‘spicy’ moments
The honest version: filler injections are not painless, but they are very manageable. The topical numbing cream and the lidocaine in the product do most of the work. Most clients describe the sensation as the spicy end of manageable – a sharp pinch that is gone in under a second. Some areas are more sensitive than others (lips tend to be the most felt; cheeks and jawline less so). If at any point you need a break, ask. Dermal filler recovery: What to avoid and when
Recovery from dermal filler is not dramatic, but there are some clear dos and don’ts that protect the result and reduce the chance of bruising or swelling sticking around longer than it needs to.
The first 24 hours
- Skip the gym. Exercise increases blood flow and can worsen swelling and bruising.
- No alcohol. It is a vasodilator and will make any bruising more pronounced.
- Avoid heat – saunas, steam rooms, and very hot showers. Heat draws blood to the surface.
- Do not press, massage, or manipulate the treated areas unless your provider specifically instructs you to.
- Sleep on your back if you can, to reduce pressure on the treated areas.
- Skip any skincare actives (retinol, acids) for 24 hours around the treatment site.
The first week
Mild swelling and any bruising typically resolve within three to seven days. Lips tend to look their puffiest in the first 24 to 48 hours, then settle significantly. Cheek and jawline filler tends to show less immediate swelling.
Your provider will ask you to avoid facials, microneedling, laser treatments, or any other facial procedures for two weeks post-filler. The filler needs time to integrate before anything touches the face.
One useful benchmark: your two-week look is your real result. Initial swelling can mask the final outcome – and occasionally make it look like more than it is. Revisit your before photos at the two-week mark. That is the starting point for assessing whether you want a touch-up.
How long do dermal fillers last?
Longevity depends on the product, the area treated, and individual metabolism. Here are typical ranges:
- Lips – 6 to 9 months. The most metabolically active area; lips break down product faster than almost anywhere else.
- Nasolabial folds / marionette lines – 9 to 12 months.
- Tear troughs – 9 to 12 months, sometimes longer.
- Cheeks and mid-face – 12 to 18 months. Structural fillers placed deep tend to last longer.
- Jawline and chin – 12 to 18 months.
These are averages, not guarantees. Clients with faster metabolisms or higher physical activity levels may find results fade sooner. Your provider will walk through a realistic maintenance schedule at your You Review based on the areas being treated.
How much does dermal filler cost?

Not if the treatment is done conservatively. The filler results that look overdone come from too much product, the wrong product for the area, or insufficient attention to overall facial balance.
