Botox or filler: Which comes first?

If you are in your late twenties or thirties and lines are your main concern, Botox is almost always the right first step. If volume loss is already part of the picture, that is when filler starts to earn its place. And if you are not sure which applies to you yet, that is exactly what your You Review – a complimentary consultation with one of our licensed practitioners – was designed for.

Key takeaways from this article

  • Botox relaxes the muscles that cause expression lines. Filler adds or restores volume. They treat different things.
  • Most providers recommend Botox first for clients in their late twenties and thirties, when movement lines are the main concern.
  • Filler becomes more relevant as volume loss gradually enters the picture, often in the mid-thirties and beyond.
  • Many clients eventually use both, at different points, for different reasons.
  • A complimentary You Review with a licensed provider is the clearest way to know which applies to you right now.

What is Botox? The treatment that stops the movement

Your face moves. All day, every day, in ways you are barely aware of: squinting at a screen, raising an eyebrow, smiling at something on your phone. Those movements are what you want. But over time, they do leave a mark. The lines that used to disappear when your face relaxed start to stick around a little longer, then a little longer, until eventually they are just there.

That is the problem Botox solves. It is a purified protein that, when placed into a specific muscle, temporarily eases that muscle’s pull. The skin above it stops folding as sharply. Lines that come from movement, which are called dynamic lines, soften. New ones are slower to form.

It works best on the upper face: forehead lines, the frown lines between the brows (sometimes called the 11s), and crow’s feet at the corners of the eyes. These are all areas where repeated muscle movement is doing the work. It does not add volume and it is not a substitute for filler. Think of it as turning down the volume on the muscle, not changing the face.

Results take three to five days to appear, with the full effect at around two weeks. Most clients rebook every three to four months. There is no downtime; most people come in on a lunch break.

What is filler? The treatment that adds back what’s missing

Dermal filler works differently. Instead of relaxing muscles, it adds or restores volume in areas where you naturally want a little more fullness or definition. That might mean enhancing the lips, restoring cheek volume or refining the jawline, depending on your goals.

Dermal fillers are gel-based injectables, most made from hyaluronic acid, a substance your skin already produces. When placed beneath the skin, they restore the volume that has gradually disappeared: plumping lips, lifting cheeks, sharpening a jawline, filling in the hollows under the eyes. Different formulations do different jobs. Softer gels sit close to the surface for lips and fine lines. Denser gels work deeper to rebuild cheekbones, chin, and jaw. Your injector chooses the product for the area, not the other way around.

Results are visible immediately, though swelling masks the final outcome for the first one to two weeks. The settled result is the one you are paying for. Depending on the product and the area, it lasts anywhere from six months (lips) to eighteen months or longer (cheeks, jawline).

Which lasts longer: Botox or filler?

Botox usually lasts around three to four months.

Most hyaluronic acid fillers last between six and eighteen months depending on the product used and the treatment area.

Although filler generally lasts longer, that doesn’t necessarily make it the better choice. The right treatment depends on what you’re trying to achieve rather than how long it lasts.

So should you start with Botox or filler?

This is the question, and the honest answer is: it depends on what your face is actually doing right now.

If the lines that bother you only show up or deepen when your face moves, Botox is the starting point. That is what it was designed for. If movement lines are your main concern, starting with Botox often creates a more natural result because it addresses the underlying cause rather than adding volume where it isn’t needed.

If volume loss is more of the story, especially thinning lips, flattening cheeks, or a jawline that has lost some of its definition, that is where filler earns its place. For most clients, this becomes relevant in the mid-thirties and beyond, though it varies significantly from person to person.

The more nuanced truth is that many clients eventually use both, not at the same time necessarily, but as different tools for different jobs at different stages. Botox tends to come first. Filler tends to layer in later. And the two work well together when the sequencing is right.

Can you get Botox and filler at the same visit?

Yes, in many cases. Some providers space them a week or two apart to make assessment easier and to let the Botox settle before evaluating what, if anything, filler needs to do. Others treat both in the same appointment, depending on the areas and the client’s goals. There is no universal rule; it is a conversation for your provider at your You Review.

What both treatments share is the goal of a result that looks like you, on a very good day. Not done. Not different. Just rested, refreshed, and still recognizably yours.

If you are in the Fort Worth area, our Fort Worth South center at Waterside Plaza has licensed injectors who offer both Botox and filler. Jennifer Flores, NP, and Natalia Jackson, RN, cover the full injectable menu and treat for balance, not maximalism.

Frequently asked questions about Botox vs filler

Botox relaxes the muscles that cause lines when your face moves. Filler adds volume beneath the skin to restore fullness or define features. They do completely different things, which is why many clients end up using both at different points, not as a package deal, but as different tools for different jobs.

In most cases, Botox first. If the lines that concern you deepen when you move your face, that is a movement problem, and that is what Botox treats. Filler belongs in the picture when volume loss is part of the story: thinning lips, flatter cheeks, a less defined jawline. Your provider can tell you which applies to you in a single conversation.

Often yes. Some providers prefer to space them slightly; others do both in one visit. It depends on the areas, the goals, and your provider’s assessment. The You Review is the place to map that out before anything is booked.

Botox at our Fort Worth South center is priced per unit, starting from $12.00 per unit (non-member), plus a one-time injection fee of $275. The total depends on how many units you need across the areas being treated, and your provider gives you a clear figure at your complimentary You Review before treatment starts. Filler starts from $599 per syringe; the number of syringes depends on the areas and the look you are after. TwistUp members get exclusive pricing on both.

Botox involves a handful of small injections over a few minutes. Most clients describe it as a quick pinch, and most areas need no numbing at all. Filler takes a little longer, and most products contain lidocaine to numb as they go in; your injector also applies a topical numbing cream. Most people describe filler as a brief pressure or pinching sensation, and the treatment is made more comfortable with topical numbing cream and lidocaine within the filler itself.

Look at the area that concerns you when your face is at rest, then when you move it. If the line deepens when you move, that is a movement issue and Botox is likely the answer. If it is there at rest, that is volume or structure, and that is where filler belongs. If you are genuinely not sure, that is exactly what the You Review is for. Your provider assesses your face, listens to what you are seeing, and tells you the real order. No obligation to book treatment on the day.

Botox or filler first? The bottom line

Botox and filler are not rivals. They are two different tools with two different jobs. Botox quiets the muscles that crease the skin. Filler restores the volume that time takes away. Most people start with Botox. Most people add filler later, when and if it becomes relevant to their face. And some people get to their You Review and find they need neither yet, just a solid skincare routine and a plan for when things change.

The point is to start with what your face actually needs right now, and to have someone qualified tell you what that is.
