Lip filler for beginners: What to know before your first treatment

For years, lip filler has held a top spot as one of the most popular and requested aesthetic treatments, but it’s also one of the treatments people are most nervous about. For many first-time clients, the concern isn’t whether lip filler works – it’s whether the result will look natural. 

The good news is that modern lip filler isn’t about dramatically changing your appearance. When performed thoughtfully, it can enhance shape, balance, and definition while still looking like you.

What is Lip Filler? (and what it isn’t)

Lip filler is a hyaluronic acid (HA) gel injected into or around the lips to add volume, refine shape, or improve symmetry. Hyaluronic acid is a substance your body produces naturally. It holds water in tissue and keeps skin supple. When used as a filler, it integrates with the surrounding tissue and moves naturally with your facial expressions.

The most common products are from the JUVÉDERM family (think Volbella, Ultra, Ultra Plus) and Restylane Kysse; each formulated with a different thickness and flexibility for different goals. A softer product gives subtle hydration and definition; a denser product adds more volume. Your provider selects the right one based on your lip anatomy and what you’re after.

Lip filler is not permanent. It dissolves gradually over six to twelve months depending on the product and your metabolism. And if you’re ever unhappy with the result, hyaluronic acid fillers can be dissolved with an enzyme called hyaluronidase, which OrangeTwist providers carry on-site.

Lip filler works best when it complements your existing lip shape and facial proportions, rather than trying to create a completely different look. It works with the shape you already have. The aim is to enhance your existing lip shape rather than completely change it.

Lip filler vs lip flips: What’s the difference?

These are two different treatments that often come up together, and clients frequently ask which one they need. The short answer: they do different things.

Lip filler adds physical volume. It fills in the body of the lip, defines the border, and can improve symmetry. Results are immediate and last several months.

A lip flip uses a small amount of Botox (or Dysport) injected into the muscle just above the upper lip. Instead of adding volume, it gently relaxes the muscle so the upper lip rolls slightly outward, creating the appearance of a fuller pout without any filler. It’s a subtler option that lasts about six to eight weeks.

Some clients do both in the same visit. A lip flip can complement filler by softening the area above the upper lip, which helps the result look more natural. Your provider will talk through what makes sense for your specific anatomy at your You Review.

What a conservative first lip filler treatment looks like

Most first-time clients at OrangeTwist use less product than they expect. That’s intentional.

How much filler is typically used

A standard syringe is 1 mL. For first-time lip enhancement, many providers start with half a syringe or a full syringe at most, depending on the starting point and goal. OrangeTwist’s philosophy is additive: start conservative, assess the result after swelling settles, and build from there. We can always add more. We cannot easily take away.

Where lip filler gets placed

Placement depends on your goals. Common areas include the body of the lip for volume, the vermilion border (the lip’s edge) for definition, the Cupid’s bow for shape, and the corners for symmetry. A good provider does not just inject everywhere-they map the treatment to your specific anatomy and ask what you actually want to see.

One important note for lip filler specifically: let your provider know your cold sore history. If you have experienced cold sores before, your provider may recommend a preventive antiviral before treatment.

Microneedling is often the better choice for acne scars, enlarged pores, and textural concerns because it creates controlled stimulation within the skin itself. For these concerns, it can produce more significant structural improvement over time.

What to expect on treatment day

Numbing and comfort

Numbing cream is applied to the lips before treatment and left to work for about 20 minutes. Most JUVÉDERM products also contain lidocaine, so the numbing continues as the filler is placed. Most clients describe the sensation as a brief pinch or pressure-spicy, not sharp-and it is well-tolerated. The injections themselves take 15 to 30 minutes.

Swelling and recovery

Lip filler swells more than filler in other areas because the lips have a rich blood supply and constant movement. Expect noticeable swelling for the first 24 to 72 hours-your result during this period is not your final result. Some clients bruise. Most are comfortable being out in public within a day or two, though full settling takes one to two weeks.

For the first 48 hours after treatment, avoid strenuous exercise, alcohol, saunas, and heat. Do not massage the lips or apply pressure. Skip makeup on the lip area for the first 24 hours. Sleep elevated if you can.

Results, maintenance, and building over time

Once swelling settles-usually within a week to two weeks-you will see your actual result. The filler integrates with the surrounding tissue and moves with your face. When lip filler is placed conservatively, the result should look balanced and natural rather than obviously treated.

Lip filler typically lasts six to twelve months. Lips dissolve filler faster than other areas because of the constant movement of the mouth during eating, talking, and expression. Most clients return every nine to twelve months for a touch-up.

Overfilled lips result from too much product placed without accounting for the natural anatomy-something that happens when providers chase trends rather than faces. Yes. A Botox lip flip can be done in the same visit as lip filler, and the two treatments work well together when planned properly. The lip flip softens the muscle above the upper lip, which can make the filler result look more natural. Your provider will assess whether combining makes sense for your anatomy and goals.

Expect swelling for one to three days, with peak swelling usually in the first 24 to 48 hours. Some bruising is possible but not guaranteed. Most clients are comfortable going about their day within 24 to 48 hours, though the lips may still look slightly fuller than the final result during this period. Two weeks is the benchmark for a settled, final result.
